Skyward Family Access Parent Portal Release Form

Complete this form to obtain a username and password to access student information online. The completed form will be routed to the appropriate school(s) for processing. You will receive an email containing directions for logging in once the school has created the Skyward Family Access account. Skyward Family Access allows you to:

  • Online Registration (update yearly forms online)
  • New student registration
  • Ability to update contact information
  • Emergency Contact Information
  • Attendance
  • Class Schedule
  • Grades in each class
  • Test Scores (including FAST Family Portal link)
  • Progress towards Graduation Requirements
  • Academic History of grades
  • Discipline Information
  • Progress Notes and Report Cards
